What is non-invasive therapeutic dentistry?
Non-invasive dentistry is the management of dental issues with complete preservation of natural tooth structure and surrounding tissues. This is possible because of the enormous advancements in therapeutic medicines to treat tooth decay and gum disease.

We integrate evidence-based medicines, personalized self-care with home products, and nutrition (the ultimate cure), into the most advanced approach: non-invasive therapeutic dentistry.

This approach emphasizes preventive care, early detection, and conservative treatment options, avoiding invasive procedures like drilling, extractions, or surgery whenever possible.

Non-invasive dentistry is aligned with the philosophical principle of "do no harm" and seeks to maintain the natural integrity of the teeth and oral tissues. It offers patients the benefits of less discomfort, elimination of post-invasive suffering, and a focus on long-term oral health.
Our Approach
Technological advancements have significantly changed the way dental issues can be treated, yet commercial dentistry has been slow to adapt. 90% of adults in the US end up with fillings, root canals, crowns and veneers when they don't need them. Invasive treatments are now indicated for less than 10% of dental issues.

Just like medicine moved on from surgeries for nearly all infections when antibiotics came around, we have too.
Non-invasive therapeutic dentistry is integrative with all the other progress we’ve made against chronic diseases.

It turns out that removing tooth structure is irrelevant for the control of tooth decay. The most common diseases, such as cavities, can now be healed rather than drilled.

Key aspects of non-invasive therapeutic dentistry

It’s a hard problem. When dental teams are focused on implants and veneers, they don't focus on the root cause of your issues. We do. And, we did not anticipate this would be nutrition. When Dr. Keeper was working for years at UCSF in search of a drug to stop tooth decay, he kept finding nutrients instead. After showing that they stopped cavity bacteria from making acid from sugar, he searched back in the forgotten literature, and found that arginine, vitamin D, and vitamin B6 prevent cavities. When he spoke to Harvard’s top researcher on gum disease, it was the same story, but with omega-3 fatty acids, curcumin, resveratrol, polyphenols, and Vitamin D. The most important thing we can do for anyone, is to talk about food.

Too much of anything creates problems. Too much water causes hyponatremia, which starts as seizures and gets worse. Even the best things in life need temperance and balance.

Fluoride prevents cavities. Normal over-the-counter fluoride toothpaste prevents 22% of cavities. Prescription strength fluoride toothpaste prevents 37% of cavities.

There is growing evidence that chronic exposure to high fluoride levels during pregnancy or early childhood may have small but measurable effects on cognitive development. Most of the studies that have shown differences are in areas that naturally have high fluoride levels in the groundwater, higher than water fluoridated for cavity prevention. And yet, there is concern that fluoride toothpaste gets swallowed so much in young children, that it could mimic this effect, or that artificially fluoridated water could have a small effect.

Fluoride varnish applied a couple times per year prevents 40% of cavities. Silver fluoride applied once per year prevents 60% of cavities. Silver fluoride twice per year stops 80% of big cavities!

We ourselves did some of the key safety studies of fluoride varnish and silver fluoride in very young children. While these studies were important for advancing policy around the world, they were somewhat performative because the doses are so tiny. The safety is obvious. The doses are infinitesimally smaller than from toothpaste or tap water, because they are only applied a couple times per year, instead of a couple times per day.

So, our philosophy is to maximize effectiveness while minimizing the exposure to fluoride - or any drug.

We accomplish this by applying tiny amounts of high concentration medicines directly to the problematic areas of the teeth and gums. The medicines absorb into the tooth, where they react to restore health, and there is hardly any left to spread into the body.

One droplet of silver fluoride has as much fluoride as a couple liters of fluoridated tap water, or a gallon of naturally low fluoride well water. We consolidate this into the tooth and remove excess. The blood levels of fluoride never go up.

We believe the risks from medicines applied directly to affected or high risk areas are far less than the risks of far larger doses of plastics, vasoconstrictors, and local anesthetics ubiquitously injected into the body by traditional dentistry.

The most effective approach to fight decay is to completely remove all sugars and overprocessed carbohydrates from the diet. But this is often impractical. Fluoride is a core part of the most efficient solutions to decay. We minimize your exposure by direct application and protect you from systemic exposure.

Our philosophy on X-rays
The American Dental Association just issued a formal evidence-based recommendation to stop using lead aprons (X-ray shields) because modern digital dental X-rays really are that safe. And, higher levels of radiation are harmful, so people are concerned.
There is still no other technology for imaging tooth roots and the surrounding bone. Yet, dental X-rays are not very accurate for diagnosing cavities. There are other options without use of radiation. Near infrared, optical coherence tomography, transillumination, and liquid contrast agents are nearly as useful. We have these technologies, and are happy to offer them as a supplement generally, and as an alternative specifically for anyone who would otherwise not access care.
